I read somewhere that only applies to the word on its own, not when it's "[Name]小姐". Not sure how true that is, but I feel like I hear "[Name]小姐" often enough in modern dramas ...?

yeah, it's definitely in Guardian at least! It's good to know not to drop the name part, though.
